Report AI Harm, Prejudice, or Safety Failure

Have you witnessed an AI system causing harm, discrimination, prejudice, or unfair treatment? Your report can help protect others and hold systems accountable. A2IS2E provides a secure, confidential channel for whistleblowers, affected individuals, and concerned observers.

We use your report to assess public interest, offer pro bono evaluation where possible, and work towards remediation. You may remain anonymous.

What to Report

We accept reports about any public-facing or consequential AI system, including:

  • Unfair denial of loans, insurance, benefits, or services
  • Discriminatory outcomes based on race, gender, language, disability, or other protected characteristics
  • Harmful or misleading advice from chatbots, health tools, or customer service AI
  • Privacy violations or misuse of personal data
  • Misinformation or manipulation campaigns
  • Unsafe deployment of AI in biosecurity, public health, or emergency response
  • Any AI behavior that caused or could cause real-world harm
  • If you are unsure whether to report, please do. We review all submissions.

Pro Bono Evaluation for Reported Harms

Where a public concern has been raised and the responsible organisation lacks budget, A2IS2E may conduct a pro bono red team engagement. This is a structured adversarial test focused on the specific harm or prejudice reported. The output is a public interest evaluation report with prioritized findings and recommendations. Where appropriate, we share the report with the company, relevant regulators, and the public.

Confidentiality and Whistleblower Protection

A2IS2E is committed to protecting whistleblowers. We do not share your identity unless you give explicit consent. All reports are stored securely with access limited to the review team. We can accept reports through encrypted channels if you require additional protection. If you are an employee of the company involved and fear retaliation, please let us know. We can provide advice on safe reporting and connect you with legal support where available.
